The Identity and Passport Service require original versions of the passport application form. These can easily be obtained from any main Post Office, and ATPI can arrange to send by post if required.
Great care needs to be taken in both completing the form, and supplying the photographs; we recommend that you read all the notes before beginning to complete the form.
